5️⃣ اللمسات الأخيرة: إضافة الـ English Subtitles للزملاء حول العالم، وتجهيز الردود على استفساراتكم العلمية.

OCT-A vs. FFA: The Ultimate Board Exam Showdown! 👁️📖

If you’re preparing for the FEBOS-R, FRCS, or any ophthalmology board exam, you know this comparison is a favorite among examiners. While FFA has been our "gold standard" for decades, OCT-A is changing the game with its depth-resolved, non-invasive capabilities.

Leakage: FFA is still king. If you need to see active leakage, you need the dye.

Layers: OCT-A wins here—it allows us to see the superficial and deep capillary plexuses separately.

Invasiveness: No needle, no dye, no nausea—OCT-A allows for unlimited repeat imaging.

🔥 Pearl 1: OCT-A detects FLOW, not leakage. FFA detects leakage and perfusion. They are complementary — not competitive. Always know which question you're asking before choosing your modality.

🔥 Pearl 2: The Outer Retinal Avascular Zone should be EMPTY on OCT-A. Any flow signal there = pathological neovascularization. This is how you spot Type 2 MNV in one look.

🔥 Pearl 3: Type 3 MNV originates from the DCP — look for intraretinal flow extending downward, always in a background of drusen. IR reflectance showing drusen + DCP flow = Type 3 until proven otherwise.

🔥 Pearl 4: Projection artifacts mimic Type 1 MNV. Always use projection-resolved algorithms and cross-check with structural B-scan before diagnosing neovascularization.

🔥 Pearl 5: Choriocapillaris flow voids on OCT-A appear BEFORE geographic atrophy is visible on FAF or structural OCT. This is the earliest detectable AMD change we have today.

🔥 Pearl 6: Ghost vessels on OCT-A after anti-VEGF = inactive treated CNV. Visible on OCT-A but no active flow. This is a treated scar — do not re-inject based on OCT-A alone without structural OCT evidence of activity.

🔥 Pearl 7: DCP FAZ is larger than SCP FAZ in normal eyes. Disproportionate DCP FAZ enlargement in DME = macular ischemia component — explains poor anti-VEGF response in some patients.

🔥 Pearl 8: In chronic CSC — always look for secondary Type 1 MNV on OCT-A. It develops silently, below the RPE, and is easily missed on clinical exam alone.
