Why do the same quality problems keep coming back?

Sometimes the defect is repaired, the NCR is answered, and the case is officially closed.

Then, a few weeks later, the same problem appears again.

This usually raises an important question:

**Did we actually solve the problem — or did we only correct what we could see?**

For me, a good corrective action should go beyond repairing the immediate defect. We need to understand why it happened, address the real cause, and later verify that our action actually worked.

Otherwise, we may simply be waiting for the same problem to happen again.

In this carousel, I’ve shared a practical way to look at repeated quality issues from a QA/QC perspective.

**From your experience, what is the most common reason quality problems keep repeating on projects?**

🔍 **5 Things to Check in an ITP Before Site Work Starts**

Having an approved ITP is important — but before starting the activity, take a few minutes to review it carefully.

Check these 5 points:

✓ Scope & References
✓ Inspection & Test Stages
✓ Acceptance Criteria
✓ Responsibilities
✓ Required Records

A practical ITP should make three things clear:

**WHAT are we checking?
WHEN are we checking it?
HOW do we know it is acceptable?**

A simple review before starting the work can prevent missed inspection points, unclear responsibilities, and documentation problems later.

🔍 Inspection Alone Does Not Guarantee Quality

Passing an inspection is important — but real quality starts long before the inspector arrives.

✔️ Approved materials
✔️ Correct procedures
✔️ Competent personnel
✔️ Proper process control
✔️ Effective inspection

All of these work together to achieve the final result.

Inspection can detect a problem.
Good quality control helps prevent it.

In your experience, where do most quality problems begin: before, during, or after ex*****on?

🔍 **EQI CASE #001 | CAN THE WORK START?**

The **Method Statement is APPROVED** ✅
The **Construction Drawing is APPROVED** ✅
The required **Material is APPROVED** ✅

But…

The **Inspection & Test Plan (ITP) is still UNDER REVIEW.** ⏳

The Construction Team says:

**“Everything else is approved. Let’s start the activity and complete the ITP approval later.”**

You are the **QA/QC Engineer.**

Would you allow the work to start?

**A — YES**
The approved Method Statement and Drawing are sufficient.

**B — NO**
The applicable ITP must be approved before starting the activity.

**C — IT DEPENDS**
The decision depends on the project requirements and approved quality controls.

👇 **What would you decide — A, B or C?**

More importantly… **WHY?**

We’ll discuss the QA/QC reasoning behind the decision.
