Woman, forty years has a small nodule over upper sternal skin. It is not adherent to bone.
USG showed a round 2.1 cms diameter hypoechoic round lesion just beneath the epidermis. It has well defined outline and onion peel appearance. No internal vascularity.
Findings consistent with subcutaneous Epidermoid cyst.
Onion peel appearance is due to layers of keratin deposits.

Illustrative case
USG image of the pancreas shows head and body. Tail of the pancreas is not appreciable.
This is an example of agenesis of pancreatic tail,a congenital anomaly.
At embryonal level pancreas is developed by two, ventral(head) and dorsal (body & tail) components.
Agenesis of dorsal component leads to the absence of pancreatic tail and accessory pancreatic duct.

Illustrative case
Male 50 years, treated for chest tuberculosis, left pleural effusion. Aspiration was done once. Treatment completed.
USG showed, at left costophrenic angle :- The layers of the pleura are thick with minimal (less than one ml) fluid between the two layers.
As the full course of ATT has been taken, this thickening is likely to resolve over period of time.
Deep breathing and blowing a balloon may help expand the the left lower lung.

Male 55 yrs, was found to have visible adrenal(Suprarenal) glands on both sides and possibly adrenal mass on right side.
However the gland measured approx 2.1*1.8 cms on right side and smaller on left side. These are normal size adrenal glands.
With improvement in resolution of USG machines these are now visualised easily and more frequently.

Illustrative case
Woman 38 years, came with vague, non localised pain abdomen. Had three children, Hysterectomy and left Oophorectomy done six years ago.
All organs except left kidney were normal. Uterus and left o***y not seen.
The normal size left kidney has a cystic area (1,2)at upper pole within capsule with dilated left ureter (3)upto lower end.(5).Surprisingly the left ureteric jet is as strong as the right one.(4)
It indicates the patient has left renal duplication with double ureter upto lower end. The surgeon saved one of the left ureter and unknowingly & inadvertently tied the lower end of the duplicate left ureter leading to hydronephrosis of the upper moeity.
Leftover left kidney is functioning well leading to strong left ureteric jet.(4).

Woman 50 years obese, came with vague abdominal discomfort.
Incidentally Both the suprerenal(Adrenal) glands were found to be enlarged (Right more than left), hyperechoic and having well defined outline.
This is classical appearance of ADRENAL MYELOLIPOMA.
This benign tumour contains fat (adipose tissue) and bone marrow elements. (Hemopoeitic tissue). The tissue generally is non-functional, producing no hormone. Most of the cases don't require any intervention.

Woman 30 yrs has undergone surgery for Urachal cyst about two months ago, by a midline infra umbilical longitudinal incision. There was intermittent pus discharge from upper end and right side of the scar.
(Urachal cyst is a rare congenital anomaly that develops as a tube in fetal remnant, Urachus, a cord like tissue connecting urinary bladder with the umbilicus)
USG showed a sinus like tract between incision point and Urachal cyst which was not completely removed.
Impression Remnant Urachal cyst with postsurgical sinus formation.
The discharge point was slightly inferior to the umbilicus.

Male 25 yrs came with pain right lumbar /RIF abdomen. No vomiting, no fever. To rule out acute appendicitis.
GB was normal. Right kidney showed minimal hydronephrosis. Pelvic separation 9-10mm.This finding gave a hint for possible right ureteric stone.
Upper ureter was prominent, mid ureter was not visualised and lower ureter showed a stone, 4.5mm at terminal end, extramural part.
Impression -Minimal right hydronephrosis with terminal right ureteric stone.
Right iliac fossa was normal, No evidence of acute appendicitis.

Male 26 years came with multiple swellings both inguinal areas,and palpable suprapubic lumps.
USG showed these to be enlarged lymph nodes in intra abdominal iliac chains and both inguinal areas.
Analysing for aetiology sc***um was scanned. Right testicle showed two small hypoechoic lesions.
Impression - Testicular malignancy with metastasic lymphadenopathy.

Male, 30 years.
Jaundice for two months, itching for one month. Deep icterus.
USG, different images show moderate intrahepatic biliary duct dilatation in both lobes, Overdistended gallbladder and solid looking proximal common bile duct with thin hair line lumen. No stone in GB or CBD.
Findings favour CBD mass(Klatskin Tumour)

Woman 25 yrs, Two months post partum, presented with pain right upper quadrant of the abdomen and vomiting. Stones in CBD were detected and biliary Stent was inserted via Endoscopy.
Now findings are
1 Partial GB distension with two microliths, 3-4 mm size.
2-Thin Stent in common bile duct and echoes of air anteriorly in non-dependent part.
3 - Multiple (four or more) microliths posterior to Stent in the dependent part of the CBD.
As these microliths are of recent origin (pregnancy), may dissolve with UDCA.
