Diagnosis- Leptospirosis
Leptospirosis, likely due to recurrent environmental exposure to contaminated water and possible direct contact with animal

Management -
1- Antibiotic therapy - the patient is started on intravenous doxycycline 100 MG twice daily given the severity of symptom and renal involvement

2- supportive care - hydration monitoring of renal function and liver function test

3- Monitoring - close observation for signs of severe complication including renal function liver failure or hemorrhagic manifestations

Outcome - after 5 day of treatment the patient fever resolve liver enzymes begin to normalise and renal function improve he is discharge after 10 days of in present care with instruction of follow up
