This child had been suffering from urticaria for 20 days.
Multiple consultations. Steroid injections. No lasting relief.
The question is not: "What medicine should I give for urticaria?"
The question is: Why did it start? When did it start? How did it start?
Only after understanding the sequence of events can we begin to understand the patient.
In this case, the prescription was Pulsatilla.
Why?
Not because it is a "medicine for urticaria."
But because the totality pointed towards it:

Some may think of Natrum muriaticum because of the 11 AM aggravation.

It is the art of understanding the entire patient.
Unfortunately, many practitioners and teachers continue to promote concepts such as "5 remedies for urticaria," "10 remedies for cough," or "best remedies for acidity."
Such approaches may be convenient for teaching, but they do not represent individualized prescribing.
Every prescription should have a logical basis.
Every symptom has a meaning.
Every patient has a story.
When the analysis is correct, the prescription often becomes obvious.

This 50-year-old lady had been suffering from recurrent painful nail infections for 3 years.

The thumb would become red, swollen, and extremely painful. Pus would form, discharge, heal, and then the entire cycle would repeat after a few months. During severe attacks, she was unable to perform household work for up to 2–3 weeks.
This condition is known as Chronic Paronychia. It is very common in women whose hands are repeatedly exposed to water, soaps, detergents, and minor trauma during daily household activities.
Normally, the cuticle forms a protective seal between the nail and the skin. Repeated irritation damages this seal, allowing moisture and microorganisms to enter. The resulting inflammation causes pain, swelling, and recurrent infection around the nail fold.
In some cases, bacteria called Pseudomonas colonize the damaged area and produce green pigments, causing the characteristic discoloration known as Green Nail Syndrome.
The challenge is not making a single attack disappear. The challenge is preventing the next one.
This patient returned after 7 months for an unrelated complaint and reported that she had not experienced a single attack since treatment.

One of the biggest mistakes students make is becoming obsessed with diagnosis while ignoring causation.

A diagnosis tells us what the disease is.
Causation often tells us why it began.
In this case, the morphology could fit several clinical diagnoses. Yet none of those labels explained the sequence of events that preceded the eruption.
When characteristic symptoms are few, the cause of the illness may become the most valuable clue in case analysis.
This is why every medical student, regardless of the system of medicine they practice, should study pathology, physiology, and disease causation in depth. Understanding disease is far more important than memorizing prescriptions.
For those who will ask:
The prescription in this case was Arsenicum album.
Why?
Arsenicum album has a strong affinity for conditions arising from exposure to filthy, contaminated, putrid, or unhealthy surroundings.
Its sphere of action includes: • Septic and toxic states • Rapidly spreading skin eruptions • Excoriated, infected, burning lesions • Conditions resulting from spoiled food, contaminated water, or unhealthy environments • Great prostration disproportionate to the pathology
In this case, the causative factor, the nature of the eruption, and the overall picture pointed towards Arsenicum album.
However, this does NOT mean every patient developing a similar eruption after exposure to contaminated water requires Arsenicum.
Another patient may need Sulphur, Rhus tox, Graphites, Mercurius, Hepar sulph, or any other remedy if the symptom totality differs.
The remedy was selected because the patient resembled Arsenicum—not because the disease had a particular name.
Homoeopathy
Pain around a toe is often dismissed as a minor local problem.
What made this case interesting was that the inflammation was not confined to the toe alone. The body's immune response had become so active that the regional lymph nodes on the same side also became painful and enlarged.
In acute inflammatory conditions, the intensity of pain is not always proportional to the size of the visible swelling. Sometimes a seemingly small lesion can produce severe suffering and significant functional disability.
Cases like these remind us that successful treatment is not merely about suppressing symptoms. The physician must understand the nature, pace, intensity, and individual expression of the disease process before selecting a remedy.
The patient's recovery was rapid, comfortable, and uneventful, with complete resolution of the acute episode.

"Which remedy?"
Calcarea Sulphurica was prescribed in this case because the patient's totality indicated it.
Homoeopathy has no disease-specific remedies. Another patient with the same diagnosis may require an entirely different medicine.
