A good outcome doesn’t always mean a good decision was made.

Sometimes the trade-off was poor. The risk was unnecessary. The reasoning was weak. You just got lucky.

The problem is that outcomes are how we tend to judge decisions.

So when a poor decision works out, it gets reinforced. We make it again. And again.

Until one day, the same decision meets different circumstances and luck runs out.

That’s why the cost of poor judgment is so often delayed.

We put serious money into getting the right systems, tools and technology in place.

Then, when it comes to the people using them, development can become a tick-box exercise.

A course completed. A policy read. A box marked done.

But even the best systems depend on the judgment of the people using them.
