Dear Parent/Carer,

Welcome back to the new school term! We hope you had a lovely summer break.

Your child is due their flu immunisation on Friday 16th October 2026.

To help us prepare, if you haven’t done so already, please complete your child’s consent form via the unique link/QR code below by day before school session. You can choose either the nasal spray (recommended for best protection) or the injectable flu vaccine if required. We have a new computer system, if you don’t want your child to receive their flu vaccine, please complete their form with a NO to avoid receiving further messages/calls.

Please note: Late forms may mean your child is unable to receive their flu immunisation at the first school session.

Remember: Flu can be a very unpleasant illness and can sometimes cause serious complications. Vaccinating your child helps protect them and reduces the spread of flu to more vulnerable family members, friends, and the wider community.

If you need support completing the form or have any questions, please contact your local School Aged Immunisation Team:

Thank you for your support in helping us keep children and families healthy this winter.

Kind regards,
