Capillary Refill time (CRT)

The ANDROMEDA-SHOCK-2 trial is a major international, randomized clinical trial published in October 2025. It evaluated 1,467 patients with early septic shock to see if a personalized resuscitation protocol targeting capillary refill time (CRT) improved patient outcomes compared to standard care. The strategy showed superiority in a hierarchical composite outcome driven largely by a shorter duration of vital organ support and shorter ICU stays, though it did not significantly reduce 28-day mortality on its own.Trial Design & ProtocolIntervention: Hourly capillary refill time (CRT) assessments paired with a step-by-step treatment algorithm over a 6-hour period.Steps: Assessed pulse pressure, fluid responsiveness, diastolic blood pressure targets via norepinephrine, bedside echocardiography, and low-dose dobutamine testing if peripheral perfusion remained abnormal.Control: Standard usual care guided by conventional international resuscitation guidelines.

RENAL TUBULAR ACIDOSIS (RTA)

Renal tubular acidosis (RTA) involves defects isolated to the renal tubules only

GFR may be normal or only minimally affected
primary problem is defective renal acid-base regulation due to impaired ability to acidify the urine and excrete acid
results in net acid retention and hyperchloremic normal anion gap metabolic acidosis (NAGMA)
may be incomplete and only develop in the presence of an acid load
occurs despite a normal or only mildly reduced glomerular filtration rate (GFR)
causes are numerous
poorly understood by many physicians
RTA is often detected incidentally through an abnormal blood workup, but some patients present with clinical features such as poor growth, dehydration, or altered mental state

ဘယ်နေရာဖြစ်ဖြစ် ကိုယ်နေခဲ့တဲ့နေရာ ကိုယ်ကျင်လည်ခဲ့တဲ့ ပတ်ဝန်းကျင် မဟုတ်တော့ဘူးဆိုရင် အရင် observation ကောငိးကောင်းလုပ်ဖို့ လိုပါတယ်
ဘယ်သူကမှ ကိုယ့်အပေါ် ကောင်းပေးဖို့ ၁၀၀% တာဝန်ရှိမနေပါဘူး
ကောင်းပေးလာရင်လည်း ကျေးဇူးတင်လိုက်ပါ

In critical care, excellence isn’t just about treating illness—it’s about consistently reviewing every aspect of patient care.
