What is the ideal resting position
of the tongue that helps in enhancing
mandibular denture stability?
Hey guys!..hope you all are doing well..
I am back and so are the animations! Trying to be more regular with animations after this long break. Feel free to ping me if you have got any suggestions for animation or if you wanna collaborate.
I was just going through this article on mandibular denture stability and hence decided to make my first animation after this long break.
Mandibular denture stability depends on two things mainly:
1. Apex of the tongue to just touch the lingual surfaces of the mandibular anterior denture teeth.
2. Also, lateral surface of the tongue touching the posterior teeth of the denture.

How to improve stability of lower dentures (Read more to find outππππ)

A concept introduced to me by Dr. Michael Gregory. According to this concept stability of lower complete dentures can be improved by omitting lower second molars.
By skipping second molars, there will be more space created for tongue and cheek to occupy.
Ultimately tongue and cheek will aid in providing stability by seating the denture towards the supporting tissues!
But how to we know how much space to be left in order to provide adequate stability?
An important rule by Dr. Michael Gregory:
Wherever possible leave a minimum of 10 mm behind the last molar tooth on a complete lower denture. Exception being marked class II relationship .

Bilateral balanced occlusion is observed when simultaneous contacts is achieved in both centric and eccentric positions.

How to apply BULL rule in order to

According to Bull rule:
Grind along the red articulating paper marks found on the inclines associated with the buccal cusps of maxillary posterior teeth and lingual cusps of the mandibular posteriors.
NOTE: The BULL rule only applies to the working side. Do not grind any other marks on the working side that are not a part of the rule.

What is a Michigan splint? ( video 1)
Michigan splint is one of the type of stabilising splint which is constructed from acrylic resin on the upper arch. Another important factor is the introduction of ramping in the splint for the canine protection during lateral excursion. Or in simple words cuspid rise is given to avoid balancing and protrusive contacts.
How should the splint equilibration look like? ( video 3)
In Michigan splint, centric relation serves as a therapeutic position which stabilizes the mandible in occlusal relations.
Apart from excluding occlusal interferences, it also helps in providing relaxation of masticatory muscles.
What happens if it is not equilibrated properly? ( video 2)
The splint must be adjusted toward centric relation at the initial appointment toward centric relation as well as at the time of each subsequent appointment. A principal objective of the initial adjustment is to provide even contacts. Check out in the last video as to what happens of there are premature contacts.

How to eliminate occlusal errors in

π¦·The Sequence to follow for the correction of occlusal errors :
(i) Restore centric occlusion,
(ii) Correct working and balancing occlusion,
(iii) Correct protrusive occlusion.
π¦·For restoring the centric occlusion the teeth are lightly tapped together on articulating paper.
π¦·Error seen during centric occlusion:
Any pair of antagonist teeth can be too long and thus hold other teeth out of contact. For correcting this error fossae on teeth are deepened by grinding, so that teeth can occlude each other.β¨.
π¦·NOTE: It is important to maintain the cusp tips for functional occlusal.

Concept of neutral zone (Read more to find outππππ)
The neutral zone is that area in the potential denture space where the forces of the tongue pressing outward are neutralized by forces of the cheeks and lips pressing inward.
These forces are developed through muscular contraction during the various functions of chewing, speaking, and swallowing, they vary in magnitude and direction in different individuals.
In neutral zone, the teeth should be positioned in such a way that the forces exerted by muscles will tend to stabilise the denture rather than unseat it.
In order to construct dentures which function properly in chewing, swallowing, speaking, etc., we must develop not only proper tooth position but also the fit and contour of the polished surfaces just as accurately and meticulously as the fit and contour of impression and occlusal surfaces.

π¦·Using bilateral manipulation to find and verify centric relation or adapted

π¦·Step one: Recline the patient all the way back
π¦·Step two: Lower the patientβs head enough so you can cradle it between your rib cage and forearm.
π¦·Step three: After the head is stabilized, lift the patientβs chin again to slightly stretch the neck.
π¦·Step four: Gently position the four fingers of each hand on the lower border of the mandible
π¦·Step five: Bring the thumbs together to form a C with each hand.
π¦·Step six: With a very gentle touch, manipulate the jaw so it slowly hinges open and closed. As it hinges, the mandible will
usually slip up into centric relation automatically if no pressure is applied. Any pressure applied before the condyles are completely seated will be resisted by the lateral pterygoid muscles.
π¦·Step seven: After the mandible feels like it is hingeing freely and the condyles seem to be fully seated up in their fossae, most experienced clinicians will assume that the mandible is in centric relation.
