🫵Construction company fined after worker dies in fall from height🫵
A construction company has been fined following the death of a worker who fell through an unsafe floor while carrying out maintenance work at a property.

HSE's investigation found that:
👉the work had not been properly planned, supervised or carried out safely
👉flooring and structural supports had been removed
👉workers continued to remove floorboards, increasing the risk of falling from height

👉Fi****ms supplier given six-figure fine after worker sustains burns👈
A fi****ms supplier has been fined £800,000 after a worker suffered burns to their hands, face and legs while disposing of pyrotechnic waste material. The material ignited after coming into contact with a hot metal disposal tray.

HSE's investigation found that the company:
🫵permitted the worker to carry out a disposal method for waste pyrotechnic composition that had no safe system of work
🫵failed to carry out a risk assessment
🫵failed to provide the worker with suitable personal protective equipment (PPE)

🫵Company fined £300,000 after worker falls through open stairwell🫵
A construction company has been fined £300,000 after a worker sustained serious spinal injuries from falling through an open stairwell and landing on concrete flooring.

HSE's investigation found:
👉the company had failed to put adequate measures in place to prevent falls from height
👉the stairwell opening had not been securely covered and the open edges were not protected

Falls from height remains one of the leading workplace fatalities in the construction industry.

🫵Food manufacturer fined £594,000 after 2 workers scalded🫵
A major food manufacturing company has been fined after 2 workers suffered scald injuries when they were replacing a leaking gasket above a boiler.
The workers were splashed with hot water which resulted in serious burns.

HSE's investigation found:
👉the company had failed to do a risk assessment
👉the work was carried out at height without adequate fall prevention measures
👉a lack of adequate supervision and monitoring procedures

🫵Companies and individual sentenced after worker dies in trench collapse🫵
Two companies and an individual have been sentenced after a worker was crushed to death when an unsupported trench collapsed at a site.
The worker had been replacing a damaged pipe at the bottom of a deep trench when suddenly the walls collapsed.
A joint investigation by local authorities and HSE found no suitable measures were implemented to prevent a trench collapse.

🫵Recycling company fined following the death of a worker🫵
A company has been fined after a worker sustained fatal injuries when a fire extinguisher exploded. The worker was killed while discharging an old fire extinguisher that had a sudden release of pressure and caused an explosion.

HSE's investigation found:
👉the company had failed to carry out a suitable and sufficient risk assessment for processing, handling and discharging fire extinguishers
👉old fire extinguishers waiting to be processed were not safely stored
👉there was no suitable restraint system in place to contain extinguishers during discharge, exposing workers to serious risk

The workbook offers a step-by-step guide, to help organisations carry out their own health and safety investigations.

HSE's most recent figures for Great Britain (2024 to 2025) show that:
👉126 workers were killed in work-related accidents
👉680,000 working people sustained an injury at work
👉59,219 injuries to workers were reported under RIDDOR

🫵Six-figure fine issued after worker is entangled in a sausage roll conveyor line🫵
A worker suffered multiple fractures, including a broken arm, when it became entangled in machinery on a conveyor line.
The worker was cleaning a sausage roll production line. Although the conveyor line had initially been switched off, it began to operate. This pulled the worker's arm into an unguarded rotating roller, causing multiple fractures.

HSE's investigation found that the baked goods company had failed to prevent access to the rotating parts of the sausage roll conveyor line.
They were fined £333,333 after pleading guilty to breaching Regulation 11(1) of the Provision and Use of Work Equipment Regulations 1998.
