Warafrin ليه مريض عنده جلطة او معرض لحدوث جلطات يتعالج بـ
؟؟Clopidogrel و واحد تانى لازم ياخد اسبرين أو
علشان نعرف السبب لازم نعرف الفرق بيـــــن:ـ
~ Arterial Thrombus:
------------------------------
@ mostly in medium-sized arteries rendered thromogenic by surface lesions on endothelial cells caused by atherosclerosis.
@ Consists mainly of PLATELETS-rich clot.
يعنى الجلطة التى تكون فى الشريان تكون بسبب احتكاك الصفائح الدموية بطبقى الخلايا المبطنة للشريان التى لم تعد ناعمة وسلسة بسبب atherosclerosisالمتراكم عليها نتيجة الــ
ولذلك يكون معظم تكوين هذه الجلطات صفائح دموية
وبالتالى فى الوقاية للمرضى المعرضين لها وايضا للعلاج اى بعد حدوثها :يجب استخدام ادوية مضادة لتجمع الصفائح الدموية مثل:
Antiplatelets: Aspirin ,Clopidogrel (Plavix® ,Clopex®,..)

~ Venous Thrombus:

@ Traggered by blood stasis,inappropriate activation of coagulation cascade ,or defect of normal hemostatic defense mechanism.
@ Consists mainly of FIBRIN- rich clots with decreased platelets.
تتكون هذه الجلطات فى الوريد بسبب مشكلة او خلل فى ميكانيزمات التجلط أو تنشيط عوامل التجلط...لذلك تكون فى معظم تكوينها من الفيبريبن
ولذلك كوقاية للمعرضين للجلطات (سرعة التجلط عالية) ,وكعلاج اذا حدثت : يجب ان يأخذ المريض دواء مضاد للتجلط مثل:

Cluster headache
ده نوع من انواع الصداع اللى بيجى للواحد مرة واحدة وهو قاعد وممكن يصيحة من النوم وبيحس كان فى حد حاطط صباعه فى عنيه الامه صعب جداااا وبيكون مصاحب بدموع بيجى للرجال اكتر من البنات
it is a primary unilateral headache involve tearing of the eyes and a stuffy nose. Attacks occur regularly for 1 week to 1 year, separated by long pain-free periods
Scientists do not know exactly what causes cluster headaches, but they appear to be related to the body's sudden release of histamine or serotonin.
Treatment of cluster headache :
Treatment does not cure cluster headaches. The goal of treatment is to relieve symptoms. The headaches may go away on their own, or you may need treatment to prevent them
help you identify your headache triggers.by making a diary :
When you get a headache,
write down the day and time the pain began.
The diary should include notes about what you ate and drank in the last 24 hours
المفروض اللى عارف انه بيجيله كلستر هديك يبعد خالص عن السيجاير والكحوليات لانها من الحاجات اللى بتزود حدوثه
Treatment mainly by:
1. Triptans, such as sumatriptan (Imitrex)
2. Several weeks of anti-inflammatory (steroid) medicines such as prednisone -- starting with a high dose, then gradually decreased
3. Breathing in 100% (pure) oxygen often relieves cluster headache for some people, particularly for frequent cluster headaches that occur at night
Other mediaction that used for prophylaxis from cluster headache is:
1.Ca channel blocker as verapamil
2.Anti eplieptic drug as valporic acid & topiramate
3.B, blocker as propranolol
4.B blocking of spheropalatine ganalion by lidocaine
