What comes first — the happy patient or the great outcome?

Trick question… they usually come together. But it often starts with this:

👉 Understanding your patient’s story
👉 Asking about their expectations
👉 Building the plan with them, not for them

When someone says:
🗣️ “I just want to garden without flaring up.”
🗣️ “I can deal with pain, but I need to sleep.”
🗣️ “Give me one thing I’ll actually do.”

That’s the roadmap.

Shared decision making isn’t fluff — it’s foundational.
Happy, heard patients = patients who show up, trust, and engage.

That’s when great outcomes follow.

The original study in the ’90s that sparked the transverse abdominis craze showed delayed activation in people with back pain.

It never claimed that retraining this one muscle would fix LBP… but the industry sure ran with that idea.

Fast forward to now:

We know back pain isn’t about the abs.
It’s about the person with those abs—their story, their context, their beliefs, their nervous system.

So yeah, maybe the gym bro can deadlift 500lbs.
But if he’s struggling with pain, maybe it’s not about strength. How come many strong people get pain? Why so many people with sarcopenia don’t?

Maybe it’s about how he relates to his body under load. Maybe it’s about load capacity. Maybe we never really know.

“If slouching was deadly, teenagers would’ve gone extinct.”

We love a good myth – it gives us simple answers to complex problems. But pain isn’t that simple, and posture isn’t the villain.

Here’s what the evidence says (Slater et al., 2019):

1️⃣ There’s no perfect posture. No “wrong” one either.
2️⃣ Spinal curves? Naturally different. Not dangerous.
3️⃣ Posture reflects mood – not just muscles.
4️⃣ Comfort matters. Explore, adjust, relax.
5️⃣ Your spine is strong. Trust it. 💪
6️⃣ Sitting won’t kill you. 🪑 Variety helps, fear doesn’t.
7️⃣ You’re unique. Posture advice should be too. 🌱

Posture ≠ Pain. Let’s stop chasing myths and start focusing on what actually works: movement, strength, rest, and mindset.

Consistency isn’t about perfection — it’s about showing up, even when the path is messy.

Too often, recovery is painted as a straight road. But the truth? It’s full of ups, downs, flare-ups, doubts, small wins, and setbacks. And that’s okay.

That’s why setting clear expectations from the start matters. Patients want to know what they’re in for — and we want to deliver.

⏳ Recovery takes weeks or months, not days.
⚡ It won’t always feel good, but progress often lags behind effort.
🪜 Small steps, done consistently, lead to big changes over time.

We also need to embrace uncertainty. The road isn’t predictable — but having a flare-up plan helps. Setbacks aren’t failures, they’re part of the process. They teach us, not break us.

And through it all, we focus on function and symptoms — not just pain. What can you do today that you couldn’t last week? What’s getting easier? That’s the real measure.

Let’s normalize the messy middle.

“Flashy Rehab vs. What Actually Works”

🚀 Futuristic, sleek, cutting-edge… but does it actually perform?

Sometimes, rehab trends feel like the Cybertruck—bold, different, exciting. But when you look closer, are they really better… or just a shiny version of something fundamental?

🏋️‍♂️ The Best Rehab Still Comes Down To:
✅ Progressive loading
✅ Movement variability
✅ Shared decisions & education
✅ Graded exposure
✅ Strength, endurance, and more

⚠️ Beware of Rehab Gimmicks!

If a new treatment method:

❌ Lacks evidence
❌ Trademarked
❌ Promises rapid results or quick fixes
❌ Sounds overly complex for no reason
❌ Prioritizes novelty over effectiveness

…then it might just be a high-tech trash can dressed up as innovation.

💡 Before jumping on the latest rehab trend, ask yourself:

👉 Is this actually better than progressive loading and education?
👉 Does this intervention have strong RCTs behind it, or just marketing?
👉 Would I use this on a family member, or does it just sound cool?
