The wound depth cannot be determined because the base is covered by slough or eschar.

✅ Clean gently with prescribed wound cleanser
✅ Assess drainage, odour, edges and surrounding skin
✅ Apply the ordered dressing
✅ Reposition and relieve pressure
✅ Document and notify the wound-care team

⚠️ Never forcibly remove eschar. Stable, dry heel eschar is generally kept intact unless otherwise ordered.

🚨 Respiratory Failure: Every Second Matters!

Recognize the warning signs: severe shortness of breath, low oxygen saturation, confusion, cyanosis, and reduced consciousness.

✅ Call for emergency help
✅ Position the patient upright
✅ Assess airway, breathing, and circulation
✅ Apply oxygen and monitor SpO₂
✅ Prepare for bag-mask ventilation or intubation
✅ Treat the underlying cause

If the patient stops breathing but has a pulse, begin rescue breathing. If there is no pulse, start CPR immediately.

⚠️ Follow your facility’s protocol and scope of practice.

A cold, painful toe with weak pulses and delayed capillary refill may indicate severe arterial ischemia. Protect the area, avoid massage or direct heat, and notify the provider immediately. Early recognition may save the limb! 🦶
