*A to Z Drug Safety Notes for NCLEX*
*A - ACE Inhibitors*: Monitor dry cough + angioedema. Check K+ and kidney function. Hold if hypotensive.
*B - Beta Blockers*: Can mask hypoglycemia. Don’t stop abruptly. Monitor HR, BP.
*C - Calcium Channel Blockers*: Monitor HR + BP. Watch for constipation, edema.
*D - Diuretics*: Monitor Potassium. Low K+ = cardiac risk.
*E - Expectorants*: Loosen secretions. Increase fluids.
*F - Fluoroquinolones*: Avoid giving with dairy, antacids, iron. Risk for tendon rupture.
*G - Glucocorticoids*: Do not stop abruptly. Risk for adrenal insufficiency.
*H - Heparin*: Monitor for bleeding. Monitor aPTT. Antidote: Protamine Sulfate.
*I - Insulin*: Regular insulin is the only insulin given IV. Monitor blood glucose.
*J - JAK Inhibitors*: ↑ risk of infections. Monitor CBC.
*K - Potassium*: Never give IV push. Must be diluted + given via pump.
*L - Loop Diuretics*: Monitor ↓K + ototoxicity. Give slowly IV.
*M - Metformin*: Hold before contrast dye + 48 hrs after. Risk for lactic acidosis.
*N - Nitrates*: Can cause hypotension, headache. Sit before giving.
*O - Opioids*: Causes respiratory depression. Antidote: Naloxone.
*P - Proton Pump Inhibitors*: Take before meals. Long term use = ↓Mg, ↓B12, bone risk.
*Q - Quinolones*: Risk for tendon rupture. Avoid in kids, pregnancy.
*R - Rifampin*: Turns body fluid orange: urine, tears, sweat. Stains contact lenses.
*S - Statins*: Risk for rhabdomyolysis. Monitor CK, liver enzymes. Take at bedtime.
*T - Thrombolytics*: Risk for bleeding. No IM injections, no BP checks on affected limb.
*U - Uterotonics*: Monitor fetal distress + uterine tone. Oxytocin = check contractions.
*V - Vancomycin*: Infuse slowly to avoid Red Man Syndrome. Monitor trough levels, kidney.
*W - Warfarin*: Monitor INR. Antidote: Vitamin K. Avoid green leafy veggies.
*X - Xanthines*: Theophylline = avoid caffeine. Monitor level. Risk for seizures.
*Y - Yastrom/Estrogen*: ↑ risk for clots, stroke, MI. Stop before surgery.
*Z - Zofran/Ondansetron*: Risk for QT prolongation. Monitor ECG if cardiac history.
