Defined as 𝙖𝙗𝙣𝙤𝙧𝙢𝙖𝙡𝙡𝙮 𝙩𝙝𝙞𝙘𝙠𝙚𝙣𝙚𝙙 ventricular wall with abnormal diastolic relaxation but intact systolic function.

Hypertrophy mainly includes 𝙐𝙥𝙥𝙚𝙧 𝙥𝙤𝙧𝙩𝙞𝙤𝙣 𝙤𝙛 𝙄𝙣𝙩𝙚𝙧𝙫𝙚𝙣𝙩𝙧𝙞𝙘𝙪𝙡𝙖𝙧 𝙨𝙚𝙥𝙩𝙪𝙢 (IVS) but not Whole left ventricule(LV) wall >>>> 𝙖𝙨𝙨𝙮𝙢𝙚𝙩𝙧𝙞𝙘 𝙝𝙮𝙥𝙚𝙧𝙩𝙧𝙤𝙥𝙝𝙮 of LV.

💥𝙃𝙮𝙥𝙚𝙧𝙩𝙧𝙤𝙥𝙝𝙮 বলতে আসলে কি বোঝায়??
আমরা যখন হাতের exercise করি তখন biceps muscle এর size কিছু দিন পর বাড়ে >> মানে biceps muscle এর fibers গুলোর size বেড়েছে নিয়মিত exercise(pressure দেয় muscle এর উপর) করার ফলে।

💥ঠিক একইভাবে যখন Heart কে বেশি কাজ করতে হয় blood pump করার জন্য তখন heart এও hypertrophy হয় যেমন 𝙖𝙤𝙧𝙩𝙞𝙘 𝙎𝙩𝙚𝙣𝙤𝙨𝙞𝙨 (aortic valve rigid হয়ে যায় >> ventricle এর systole এর সময় aortic valve ঠিক মতো open হয় না >> aortic opening narrow হয় >>> এখন এই narrow opening দিয়ে যদি blood pump করতে চায় তাইলে বেশি কাজ করা লাগবে >>LV hypertrophy.)

💥HCM এবং aortic stenosis দুই ক্ষেত্রেই LV hypertrophy হয় >> কিন্তু HCM ছাড়া অন্য যে কোন কারণে যদি hypertrophy হয় সেটা পুরো LV এর wall এই হয় >> HCM তে অনেক ক্ষেত্রেই শুধু মাত্র Upper portion of IVS এ hypertrophy হয়....তারমানে Asymmetrical hypertrophy হয় আর Aortic stenosis বা Hypertension এ Whole LV এ Hypertrophy হয়.

🅲🅰🆄🆂🅴🆂:

1.
𝐅𝐚𝐦𝐢𝐥𝐢𝐚𝐥 (Autosomal Dominant :mutation of genes that encode sarcomere protein complex like myosin,troponin,myosin binding protein >>>যার ফলে theses contractile unit can not contract Properly >> increase in myocyte stress(যেমন দূর্বল মানুষ কে যদি ৪০ কেজি ওজন তুলতে বলা হয় তার কাছে অনেক কষ্ট(pressure) মনে হবে.অন্যদিকে একজন সবল মানুষের কাছে ৪০ কেজি ওজন কিছুই না, সহজেই তুলতে পারবে) >> myocyte stress বেশি তাই myocyte মনে করতেছে to pump adequately আরো অনেক বেশি contractile proteins form করা লাগবে >> myocyte size increased >> Hypertrophy.

যখন ventricle এ pressure বেশি থাকবে এবং hypertrophy হবে তখন এই pressure myocardial fibroblast কে stimulate করবে >>fibrosis >>>ফলে HCM তে systole অনেক strongly হবে(যেহেতু muscle mass increased) কিন্তু diastole ঠিক মতো হবে না (fibrosis এর জন্য)

🅟🅐🅣🅗🅞🅟🅗🅨🅢🅘🅞🅛🅞🅖🅨:

HCM দুই ধরনের হয়
1:
𝐇𝐂𝐌 𝐰𝐢𝐭𝐡𝐨𝐮𝐭 𝐋𝐕 𝐨𝐮𝐭𝐟𝐥𝐨𝐰 𝐨𝐛𝐬𝐭𝐫𝐮𝐜𝐭𝐢𝐨𝐧(মানে Hypertrophy Interventricular septum কে শুধু involve করে নাই, LV এর অন্য যায়গায় Hypertrophy হয়েছে) ::

📢 Myocyte Hypertrophy তে Muscle Fiber and সাথে থাকা Bundle of HIS,Purkinje fiber এর orientation নষ্ট হয় যার ফলে SA node থেকে আসা Electrical impulses ঠিকমতো Transmitted হয় না Ventricle এ>>>>> >> 🆅🅴🅽🆃🆁🅸🅲🆄🅻🅰🆁 🅰🆁🆁🅷🆈🆃🅷🅼🅸🅰.

📢 Arrhythmia তে Heart rate অনেক বেশি হয় তাই Cardiac cycle এর Duration অনেক কমে যাবে (diastolic time +systolic time) >> তাহলে diastolic টাইম decreased >>যেহেতু Diastolic time কম ventricle filled হবার জন্য কম সময় পাবে >>আর যেহতু HCM তে 𝙙𝙞𝙖𝙨𝙩𝙤𝙡𝙚 ঠিক মতো হয় না তাই 𝙚𝙣𝙙 𝙙𝙞𝙖𝙨𝙩𝙤𝙡𝙞𝙘 𝙫𝙤𝙡𝙪𝙢𝙚 কম হবে>> ventricle এ blood যেহেতু কম 𝙘𝙖𝙧𝙙𝙞𝙖𝙘 𝙤𝙪𝙩𝙥𝙪𝙩 ও কম হবে এবং নিচের sign, symptoms দেখা দিবে

🎙️Decreased Cardiac output >>> lack of cerebral perfusion >> transient loss of consciousness>>> 🆂🆈🅽🅲🅾🅿🅴.

🎙️decreased cardiac output >> less blood in Coronary Artery >> ischemia >> less oxygen in coronary blood >> 🅰🅽🅶🅸🅽🅰...
📢 myocyte hypertrophy >>> যেহেতু hypertrophy তে cell এ বেশি contractile Units থাকে >> increased contraction >>increased need of ATP to contract >>> increased need of Oxygen but decreased Supply >>>🅰🅽🅶🅸🅽🅰.

📢 Diastolic impairment (due to fibrosis)>> যেহেতু diastole হবে না ঠিকমতো তাই Left atrium থেকে Left ventricle এ blood কম যাবে >>> তাহলে left attial pressure বাড়বে>>>increased Pulmonary vein pressure >> Pulmonary edema >>> 🅳🆈🆂🅿🅽🅴🅰....

Hypertrophy Mainly interventricular septum এর Upper part কে involve করে >>>যার ফলে LV systole এর সময় LV narrow হয়ে যায় >> এই Narrow space দিয়ে Rapidly blood flow হতে থাকে >>>> rapid flow এর কারনে Venturi force created হয় এবং এই Force anterior leaflet of mitral valve(MV) কে attract করে imterventricular septum এর দিকে >>>Left ventricular 𝙊𝙪𝙩𝙛𝙡𝙤𝙬 𝙩𝙧𝙖𝙘𝙩 𝙤𝙗𝙨𝙩𝙧𝙪𝙘𝙩𝙞𝙤𝙣 হয় >>> Cardiac output >>> 🆂🆈🅽🅲🅾🅿🅴..

🄻🄸🄶🄷🅃 🄷🄴🄰🄳🄴🄳🄽🄴🅂🅂 in standing position (in standing position 𝙫𝙚𝙣𝙤𝙪𝙨 𝙍𝙚𝙩𝙪𝙧𝙣 𝙞𝙨 𝙡𝙚𝙨𝙨>>>যদি কম blood থাকে ventricle এ তাহলে >>Septum এবং Anterior leaflet of MV আরো কাছাকাছি আসে >>> more obstruction.

অন্য দিকে যদি কোন কারণে যদি 𝙫𝙚𝙣𝙤𝙪𝙨 𝙧𝙚𝙧𝙪𝙣𝙨 বেশি হয়>>>LV এ বেশি blood আসবে>>>ventricle এ blood যত বেশি হবে,ওই বেশি blood septum এবং leaflet of MV কে তত দুরে ঠেলে দিবে >>> LV outflow obstruction হবে না >>>increased Cardiac Output)

ℂ𝕒𝕣𝕕𝕚𝕠𝕞𝕪𝕠𝕡𝕒𝕥𝕙𝕚𝕖𝕤 are Heart Muscle disorders that cause impairment of heart muscles contractile functions.

Anatomical appearance and physiologically তিন ভাগে ভাগ করা যায়:

1.Dilated Cardiomyopathy

2.Restrictive Cardiomyopathy

3.Hyperthrophic Cardiomyopathy.

আজকে 𝘿𝙞𝙡𝙖𝙩𝙚𝙙 𝘾𝙖𝙧𝙙𝙞𝙤𝙢𝙮𝙤𝙥𝙖𝙩𝙝𝙮 Discuss করা যাক।

💥🅓🅘🅛🅐🅣🅔🅓 🅒🅐🅡🅓🅘🅞🅜🅨🅞🅟🅐🅣🅗🅨 তে ventricle dilated হয় এবং ventricle এর contractile function লস হয় Heart muscle degeneration এবং fibrosis of heart muscle এর কারণে । কেন Heart muscle degeneration, Fibrosed হয় একটু পরেই ব্যাখ্যা করব।

🅒🅐🅤🅢🅔🅢

1.Most of the case এ 𝙞𝙙𝙞𝙤𝙥𝙖𝙩𝙝𝙞𝙘.

2.𝙁𝙖𝙢𝙞𝙡𝙞𝙖𝙡

3.𝙄𝙣𝙛𝙡𝙖𝙢𝙢𝙖𝙩𝙞𝙤𝙣 ( যেমন 𝘼𝙘𝙪𝙩𝙚 𝙫𝙞𝙧𝙖𝙡 𝙢𝙮𝙤𝙘𝙖𝙧𝙙𝙞𝙩𝙞𝙨 caused by 𝘾𝙤𝙭𝙨𝙖𝙘𝙠𝙞𝙚 virus এ immune meditated response হয় > macrophage, Activated T lymphocyte immensely activated হয় > virus কে destroy করতে গিয়ে Heart muscle ও destroyed হয়, এটাই Degeneration of heart muscle > যেহেতু Heart এ virus এর বিরুদ্ধে inflammation শুরু হয়েছে এবং tissue damage হচ্ছে তাই damage healing এর জন্য fibrosis হবে).

4. 𝘾𝙤𝙣𝙣𝙚𝙘𝙩𝙞𝙫𝙚 𝙩𝙞𝙨𝙨𝙪𝙚 𝙙𝙞𝙨𝙚𝙖𝙨𝙚(Defect in elastin, collagen)

5.𝙥𝙚𝙧𝙞𝙥𝙖𝙧𝙩𝙪𝙢 𝘾𝙖𝙧𝙙𝙞𝙤𝙢𝙮𝙤𝙥𝙖𝙩𝙝𝙮 (Cardiomyopathy during last trimester to 6 months after Child birth is Called peripartum Cardiomyopathy which cause is unknown)

6.𝙎𝙖𝙧𝙘𝙤𝙞𝙙𝙤𝙨𝙞𝙨(some chemical stimulus initiate immune response that causes accumulation of inflammatory cells >leads to degeneration of heart muscle).

7. 𝘾𝙝𝙧𝙤𝙣𝙞𝙘 𝙖𝙡𝙘𝙤𝙝𝙤𝙡 𝙘𝙤𝙣𝙨𝙪𝙢𝙥𝙩𝙞𝙤𝙣 ( Ⓕⓘⓡⓢⓣⓛⓨ
mitochondrial oxidative function impaired হয় >less production of NADH,FADH > production of ATP > impairment of Heart muscle contraction যেহেতু muscle contraction এ ATP লাগে,

ইথানল intracellular Calcium এর সাথে bind করে > decreased level of Intracellular Calcium > impairment of muscle contraction,

Alcohol directly Heart muscle এর apoptosis করে >Degeneration of heart muscle).

10.𝙈𝙪𝙨𝙘𝙪𝙡𝙖𝙧 𝙙𝙮𝙨𝙩𝙧𝙤𝙥𝙝𝙮 তে Gene mutation এর ফলে contractile protein production কম created হয় >impairment of contractile function.

Both ventricle affected হয় বেশিরভাগ ক্ষেত্রে।

1.𝐝𝐞𝐜𝐫𝐞𝐚𝐬𝐞𝐝 𝐂𝐚𝐫𝐝𝐢𝐚𝐜 𝐎𝐮𝐭𝐩𝐮𝐭 >> decreased perfusion of tissues >> decreased production of ATP by muscle of Whole body >> >>🄳🄴🄲🅁🄴🄰🅂🄴🄳 🄲🄰🅁🄳🄸🄰🄲 🄾🅄🅃🄿🅄🅃

আবার যেহেতু RA এ pressure বেশি তাই inferior venacava(IVC) RA এ drain কর‍তে পারে না >> increased IVC pressure >> এজন্য hepatic vein এ pressure বেশি >>liver থেকে blood drain হতে পারে না তাই 🄷🄴🄿🄰🅃🄾🄼🄴🄶🄰🄻🅈.

IVC এ pressure বেশি হওয়ায় পুরো body তে blood congested >>🄰🅂🄲🄸🅃🄸🅂 ,🄿🄴🅁🄸🄿🄷🄴🅁🄰🄻 🄴🄳🄴🄼🄰.

কিভাবে??

1.
LV hypertrophy তে LV Muscle size increased হয় >>> needs more oxygen to produce adequate ATP for Muscle contraction.

LV এ pressure বেশি যার ফলে LV এর wall এ জোরে pressure দেয় >>sub- Endocardium এর blood vessels গুলো constricted হয়ে যায় >>> decreased Oxygen supply to myocardium.

মানে একদিকে Myocardial oxygen demand বেড়ে যায় অন্য দিকে myocardial oxygen supply কমে যায়>>>🄰🄽🄶🄸🄽🄰.
