A young man of 22 yr presented with fever, cough and chest pain. Chest X-ray showing right upper lobe consolidation. Take a note of the contour of the fissure which is clearly seen here.

MRI showing fluid level in the pituitary. The patient may present with severe headache, cranial nerve palsies, visual field defects and hypotension. The diagnosis is pituitary apolexy. Management is usually conservative unless in deteriorating consciousness level and worsening visual field defects in which case neurosurgical intervention is required.

One of my happiest moments are the moments, when someone, who i have the privilege to help academically, pass in a difficult exam. Congratulations to those who have passed SCE Endocrinology in this diet.
If you're hearing a systolic murmur all over the precordium, search carefully for radiation in the neck. You may have a case of aortic stenosis. Look for slow rising pulse and the ejection systolic murmur loudest in the aortic area.
SCE Endocrinology exam will take place on 13th November, 2024. For the interested candidates, i am going to arrange 7 online classes. The classes will have a focused summary of the important topics and discussion on frequently encountered questions. Interested candidates please let me know.

During fundoscopy, don't forget to tell your patient to look straight into the light. Look carefully for exudates and edema around the macula to diagnose diabetic maculopathy. Causes of visual impairment in a patient with diabetes are:
Cataract
Diabetic maculopathy (CSME)
Retinal detachment
Vitreous hemorrhage
Glaucoma
CRAO
