Psychotropic medicines can be valuable and sometimes essential, but their use in general hospitals often begins in moments of clinical uncertainty, including agitation, delirium, distress, insomnia, nausea or behavioural change. If the underlying problem, intended treatment target and duration are not clearly defined, a short-term prescription can readily persist beyond the acute episode. Sedation, falls, aspiration, anticholinergic effects, movement disorders and drug interactions are particular concerns in older people and those with cognitive impairment. Recent Austin audit data suggest that close to half of patients receiving an antipsychotic during the audited period were new starts. This does not imply that those prescriptions were inappropriate. Rather, it highlights hospitalisation as an important point at which psychotropic exposure is created, changed and potentially perpetuated.

Concerns identified by the Aged Care and Disability Royal Commissions reframed psychotropic overuse and chemical restraint as matters of patient safety, consent and human rights, informing the 2024 Psychotropic Medicines in Cognitive Disability or Impairment Clinical Care Standard. Using clinical cases and local prescribing data, this Grand Round will examine what good psychotropic stewardship looks like on the medical ward. It will consider assessment of the causes of behavioural change, non-medication strategies, clear indications and treatment targets, individualised assessment of benefits and risks, and plans for review, cessation and communication across transitions of care. The session will also present Austin’s Psychotropic Management Plan, deprescribing initiatives and emerging dashboard-based approach to monitoring practice.
