Venous thromboembolism (VTE), encompassing deep vein thrombosis (DVT) and pulmonary embolism (PE) is a major health problem. It affects 1 in 1000 people, increasing in incidence with increasing age. It remains one of the most common cause of preventable hospital-related death despite more attention being paid to appropriate VTE prophylaxis in, and after, hospital admissions. The treatment of VTE has been revolutionised over the last decade with the advent of DOACs. However, despite their ease of use and relative safety compared to warfarin, there remains a proportion of patients who have major bleeding problems related to anticoagulation. Anticoagulants are a high-risk medication and there continues to be incidents related to incorrect choice, duration or dose. We will be presenting on the medical management of VTE including landmark papers that have been published that inform our practice. We will then be highlighting innovations and practices we have put in place locally to improve VTE prevention, treatment and management of anticoagulants.
