📌 What is H. pylori?
Helicobacter pylori is a Gram-negative, microaerophilic, spiral-shaped bacterium that colonizes the gastric mucosa. Discovered by Barry Marshall and Robin Warren in the early 1980s, it’s now well-established as a major cause of several gastric pathologies.

📊 Epidemiology:
• Affects over 50% of the global population.
• Higher prevalence in developing countries.
• Often acquired during childhood, persisting lifelong if untreated.

🦠 Microbiology:
• Gram-negative, curved/spiral rod
• Motile — has 4-6 unipolar flagella
• Microaerophilic — prefers low oxygen (5-10%)
• Urease-positive — hydrolyzes urea to ammonia, neutralizing gastric acid.

🩺 Pathogenesis:
1. Survives stomach acidity via:
• Urease enzyme → converts urea to ammonia
• Flagella → burrows into mucous layer
2. Adheres to gastric epithelial cells
3. Releases cytotoxins:
• CagA (cytotoxin-associated gene A) → associated with cancer risk.
• VacA (vacuolating cytotoxin A) → induces cell damage.
4. Triggers chronic inflammation → gastritis, ulcers.

• Asymptomatic (most common)
• Chronic gastritis
• Peptic ulcer disease (gastric & duodenal ulcers)
• Gastric adenocarcinoma
• MALT lymphoma (mucosa-associated lymphoid tissue)

🧪 Diagnosis:
1. Non-invasive:
• Urea breath test (high sensitivity & specificity)
• Stool antigen test
• Serology (IgG) — not useful post-treatment
2. Invasive (endoscopic biopsy):
• Rapid urease test (CLO test)
• Histology
• Culture (difficult but definitive)
• PCR (for virulence factors & resistance genes)

💊 Treatment (Eradication Therapy):
First-line:
• Triple therapy (14 days):
• Proton Pump Inhibitor (PPI)
• Clarithromycin
• Amoxicillin (or Metronidazole if penicillin-allergic)
If resistance high / treatment fails:
• Bismuth-based quadruple therapy
• PPI
• Bismuth subsalicylate
• Tetracycline
• Metronidazole

• Increasing resistance to clarithromycin, metronidazole, and levofloxacin is a major concern.

A heart attack, or myocardial infarction (MI), occurs when blood flow to a part of the heart muscle is abruptly blocked, typically by a blood clot, causing tissue damage. The most common cause is atherosclerosis, where plaques made of fat, cholesterol, and other substances build up in coronary arteries. When a plaque ruptures, it can trigger clot formation, obstructing blood flow.
Classic symptoms include chest pain or discomfort, radiating to the jaw, neck, arm, or back, shortness of breath, nausea, sweating, and dizziness. However, atypical presentations are more frequent in women, elderly patients, and diabetics.
Diagnosis involves electrocardiography (ECG), cardiac biomarkers like troponins, and imaging studies. Immediate treatment focuses on restoring blood flow, often through percutaneous coronary intervention (PCI) or thrombolytic therapy. Adjunctive medications include antiplatelets, anticoagulants, beta-blockers, and statins.
Recent advances highlight the role of high-sensitivity troponin assays for earlier detection and dual antiplatelet therapy (DAPT) for secondary prevention. Emerging research into cardiac regeneration, genetic risk profiling, and inflammatory biomarkers is refining risk stratification and management. Lifestyle modifications and cardiac rehabilitation remain vital for long-term prognosis, reducing recurrence risk and improving quality of life post-MI.

Widal Test is an agglutination test that detects the presence of serum agglutinins (H and O) in the serum of patients with typhoid and paratyphoid fever.
● It was developed by Georges Ferdinand Widal in 1896.
● The fundamental principle of the widal test is that if there are homologous antibodies in the patient's serum, it will react with the respective antigen in the reagent and produce visible agglutination on the test card and agglutination on the tube. The antigens used in the test are the "H" and "O" antigens of Salmonella Typhi and the "H" antigen of S. Paratyphi. Paratyphoid "O" antigen is not used as it cross-reacts with typhoid "O" antigen due to sharing factor 12. "O" antigen is a somatic antigen and "H" antigen is a flagellar antigen
● Preparation of Widal antigens Bacterial suspension H is prepared by adding 0.1% formalin to a 24-hour culture broth or a saline suspension of an agar culture. For the preparation of O suspensions of bacteria, organisms are grown on phenol agar (1:800) to inhibit flagella. Standard smooth strains of the organism are used; S Typhi 901, O and H strains are used for this purpose. The growth is then emulsified in a small volume of saline, mixed with 20 times its volume of alcohol, heated to 40°C to 50°C for 30 minutes, and centrifuged. The antigens are treated with chloroform (preservative) and appropriate dyes are added for easy identification of the antigens.

Magnesium ammonium phosphate crystals (formerly triple phosphate or struvite) appear mainly in alkaline urine (pH>8). They are colorless, pleomorphic and do not appear in healthy individuals, always being associated with infections by ureolytic bacteria.
