These features describe PELLAGRA.

Isoniazid can cause Vitamin B6 deficiency ➡️ which presents with cheilitis, glossitis, neuropathy, and sideroblastic anemia

Isoniazid can also interfere with tryptophan metabolism ➡️ Niacin (Vitamin B3) is synthesized from tryptophan

Niacin deficiency leads to pellagra ➡️ Dermatitis (Casal necklace: circular rash on the neck around dermatomes C3 and C4), diarrhea, and dementia (hallucinations, anxiety, neurologicvsymptoms, etc.)

No matter what your suspicions are: If you have an X-ray, MRI, CT scan, ultrasound, esophagram, capsule endoscopy, etc., interpreting these images correctly is of the utmost importance.

This is where understanding the criteria for a diagnosis matters. Let's use The diagnosis of Major Depressive Disorder

According to DSM-5: MAJOR DEPRESSIVE DISORDER

Five (5) or more of the symptoms listed below for at least 2 continuous weeks with at least one of the symptoms as inability to feel positive emotion or inability to enjoy experiences that are normally pleasurable

- Inability to feel positive emotion
- Inability to enjoy experiences that are normally pleasurable
- Insomnia or prolonged sleep
- Feelings of guilt
- Loss of energy
- Diminished concentration
- Loss of appetite
- An increase in aggressive behavior or a loss of initiative
- Thoughts and a plan for harming oneself and/or others

Now notice that I did not include substance abuse, past medical history, or laboratory diagnostics. In practice, you rule these issues out. For answering questions, you learn the criteria.

Regardless of the cause of the symptoms of depression, if you meet five or more of those symptoms, proceed to treatment.

Your objective is to bring the patient to baseline or their usual state. The critical information begins with vitals

VITALS

Vitals reveal if a patient is unstable. The human body is resilent so when the body becomes unstable there is a serious condition.

To give context imagine a person with a rash. A rash does not change the vitals and it can be self-limiting or resolved over time with a topical cream.

Now imagine the same person with a gunshot wound. The person is hemorrhaging which would lower the blood pressure. If the person loses too much blood they will suffer from organ failure. The blood pressure was your first warning sign.

Now ask yourself this which is more important the rash or gunshot wound? The vitals tells you where to go to first.

PAST MEDICAL HISTORY

Past medical history is essential for preventing contraindications. I previously posted a THIS or THAT on insomnia. The treatment was proper sleep hygiene but what about the past medical history of bipolar disorder. Sleep restriction is part of sleep hygiene BUT is not appropriate if the person with insomnia has bipolar disorder. Sleep restriction would induce mania in a person with insomnia and bipolar disorder.

Stanford A dissection - Involves the ascending aorta
Treatment - Immediate surgery

Stanford B dissection - Originates distal to the left subclavian artery
Treatment - Watchful waiting and medical therapy

Proper sleep hygiene!

Exercise but 3-4 times before bedtime is part of the criteria for good sleep hygiene.

Sleep restriction is also part of the criteria for good sleep hygiene BUT not in bipolar patients. Sleep restriction in bipolar patients may induce mania.

So the only correct answer is: EXERCISE BUT 3-4 HOURS BEFORE BEDTIME
