The AHA has a new classification of guidelines changing the definition of high blood pressure – also called hypertension – to begin when measurements show a top number of 130 or a bottom number of 80. That changes from 140/90 ICD-10-CM uses only a single diagnosis code for individuals who meet criteria for hypertension and do not have comorbid heart or kidney disease. Medical necessity is a United States legal doctrine, related to activities which may be justified as reasonable, necessary, and/or appropriate, based on evidence-based clinical standards of care. Health care services or supplies needed to diagnose or treat an illness, injury, condition, disease or its symptoms and that meet accepted standards of medicine. Medical necessity documentation, or lack of it, is one of the most common reasons for claim denials. For a service to be considered medically necessary, it must be reasonable and necessary to diagnosis or treat a patient's medical condition.
