Today I’m at St. Anthony’s Hospital again! Since this is only my second day, I am still training on what I will be doing for their Transitions of Care program. This program is a targeted program for patients to prevent readmission to the hospital. I will be helping head the program as well as precepting APPE student who are on the Transitions of Care rotation at St. Anthony’s Hospital. I will also be helping out as needed in the outpatient pharmacy answering questions, counseling patients and verifying discharge prescriptions!

Started at St. Anthony’s this week as a part of the second half of my residency program!
  ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀
For the first 6 months I have been at an ambulatory care clinic and this week I started at the hospital. What I’ve loved about my program is the diverse opportunities and work environments. When I first heard about “community-based” residency programs, I originally thought I would be just working in the community pharmacy for most of my days. On a typical week at the Denver Division, I spend 1-2 days at my community store, 1 day teaching at Regis University, 1 days at my clinic site and 1-2 days in the rotation I’m currently completing. I’ve learned so much in the past 6 months and I can’t wait to start my journey at St. Anthony’s participating in their transitions of care program and precepting APPE students!

Wednesday Takeover: Today I’m performing CMRs’ for the 5 stores I manage as well and catching up on project. Normally, I would be working out of the Division office, but due to COVID19 I’m working from home. I’ve been asked time and time again if I feel as though my relationship with my residency team has been diminished due to not being in person. While, I would love to be in person with my team, but I honestly talk to my team daily. Whether it’s talking residency, future career goals or just catching up with one another, I don’t think my support system has been affected at all! Love the Denver team with all my heart!!

The extensive leadership and management training we receive as residents is something very unique about our program. Today I’m working from home during my 6-week Leadership and Management learning experience. So far I’ve help create material for a technician training course, residency interviews and residency accreditation standards. We also work closely with the Division Pharmacy Manager and Patient Care Services Manager during this rotation. Can’t wait to see what else this rotation has in store for my co-resident and myself!

I’ve had the pleasure of helping immunize fellow healthcare workers against COVID19 for the past week in my residency. As a resident in the Denver Division, we typically spend 1-2 days at our community site. This has been filled in with helping give vaccines for the past week and a half. It’s incredibly exciting and inspirational to see the amount of work our pharmacists, student pharmacists, technicians and other staff are doing to ensure everyone in eligible is being vaccinated! I’m honored to be a part of it!

An alumna of University of the Pacific Thomas J. Long School of Pharmacy Austin-Phong currently serves as the PGY1 Community Pharmacy Resident at Albertsons Companies PGY1 Community-Based Pharmacy Residency Program in Pleasanton, California.

Dr. Nguyen states “I am a huge advocate for mental health. The use of pharmacogenomics in guiding mental health treatment can greatly reduce symptomatology and increase quality of life. Today is my big day! I have a huge Continuing Education presentation to present with all of the faculty and residents from the University of Maryland Medical Center. I know I’d be much more nervous if it were in person. However, due to COVID-19 this is what it’s looking like!
The Maryland program has these two required CE presentations. This one is the Pharmacotherapy Rounds rounds presentation at UMD and the other one is the Albertsons Companies called Grand Rounds. There are opportunities through the year to present more CEs if you want. Not to mention teaching lectures in front of students too. My presentation today is all about coronavirus and the vaccine. My research project is also centered around the coronavirus vaccine. I’m really glad that as a part of this residency we get to choose our topics for our projects and presentations because this is a HOT one! The University of Maryland is the institution we partner with for our teaching certificate and other presentations such as this. Due to COVID, I haven’t really been to campus. Just to pick up my official Dr. Cherokie Dyer sweater that matched the other University of Maryland Medical center residents.

Hi, I’m Cherokie! I’m one of the Community residents from the Maryland site. I live in Towson, MD with my husband and puppy, and my main community site is in Parkville, MD. I’m from South Florida and I went to school at the University of Florida College of Pharmacy in Gainesville!

1. I got married this year during the pandemic!
2. I have a passion for preventative medicine, nutrition, and helping minority populations!
