DEPTH CONTROL INDEX FOR VENEER PREPS

I saw these digital indexes the other day and decided to give them a try.

They look nice, but they only give us information about the depth in the center of the buccal surface.

As we need to control the depth throughout the buccal surface, more depth control indexes must be made to give us that information. Maybe using less fancy but simpler designs.

PAIN MANAGEMENT: clinical guide

Acute dental pain can be one of the worst pains experienced by human beings and that is why it is important that the dentist knows how to manage this type of situation.

🟢 Acute pain (mild/moderate): Paracetamol.

🟡 Acute pain (mild/moderate) with an inflammatory component: Ibuprofen or Diclofenac.

🔴 Acute pain (moderate/severe): Ibuprofen + Paracetamol or Clonixin.

🥵 Does pain persist? Associate Codeine or Tramadol to previous therapy. .

🆘 Does pain still persist? Consider prescribing a more potent opioid drug.

More important than controlling the pain is eliminating its source by treating the underlying problem.

REMEMBER, pain is not treated with antibiotics. These are only used in specific situations.

ORAL CANDIDIASIS - CLINICAL GUIDE

In the initial examination, it is important not to forget to inspect the soft tissues and look for changes or pathologies like oral candidiasis.

30 to 50% of patients have the Candida albicans fungus, which makes them susceptible to developing oral candidiasis if there is an alteration in the host's immune status or a change in the environment of the oral mucosa.

- Uncontrolled systemic diseases;
- Non-permeable ostium (difficulty in draining and naso-sinusal ventilation);
- Acute or chronic sinusitis (refer to an otolaryngologist);

- Radiotherapy;
- Bisphosphonates.

🇬🇧 The BruxChecker is a thin vacuum pressed-sheet fabricated using a simple and quick method. It’s application in the patient’s mouth causes no apprehension on the patient’s part because the device is not invasive. Patients are simply instructed to wear the device immediately before going to bed and to remove it upon waking up in the morning. The occlusal surface was painted with a red dye dissolved in shellac-ethanol solution in order to visualize the grinding facets.

🇵🇹 O BruxChecker é uma goteira fina prensada a vácuo, fabricada usando um método simples e rápido. A sua colocação na boca do paciente não causa apreensão por parte do mesmo já que o dispositivo não é invasivo. Os pacientes devem colocar o dispositivo imediatamente antes de irem dormir e removê-lo ao acordar. A superfície oclusal é pintada com um corante vermelho para permitir a visualização das facetas de desgaste.

🇬🇧 Schneider membrane perforation:

➡️ Less than 5mm: spontaneous healing occurs. The membrane is pleated when raised.
➡️ Between 5 to 10mm: suture with resorbable suture, occlude with resorbable membrane, peel off edges of perforation; apply A-PRF membrane (Advanced platelet-rich fibrin).
➡️ More than 10mm: cover with lamellar bone strip (collected from the posterior region of the mandible) and an A-PRF membrane.

➡️ Allograft (bone from a cadaver): the bone has some osteogenic cells, however it depends a lot on the cadaver from where the bone is collected.
➡️ Xenograft (bone from an animal): Its use carries a series of risks, namely the transmission of diseases.
➡️ Alloplastic material: they are materials prepared at laboratory level and for this reason the results are not so good (eg bioactive glass).
➡️ Biological material: they are osteoinductive materials with the ability to transform undifferentiated cells into cells with osteogenic capacity (eg plasma rich in growth factors).

➡️ Xenoenxerto (osso de um animal) A sua utilização acarreta uma serie de riscos nomeadamente a transmissão de doenças.
➡️ Material aloplástico: são materiais preparados a nível laboratorial e por este motivo os resultados não são tão bons (ex. vidro bioativo).
➡️ Material biológico: são materiais osteoindutores com a capacidade de transformar células indiferenciadas em células com capacidade osteogénica (ex. plasma rico em fatores de crescimento).
