Good news!

This is to inform you that the deadline for the completion of deficiencies of the graduating students is extended until June 15, 2022.

To check the list of candidates for graduation with deficiencies, please proceed to your college immediately. Those who are not on the list are advised to see their evaluator at the Registrar's Office.

To Our Valued Students:

PhilHealth Registration Forms are now available at the OSAS Office!

This is in compliance to CHED-DOH Joint Memorandum Circular No. 2021-004, students who prefer and/or are allowed to attend limited face to face classes shall be registered with PhilHealth or with an equivalent medical insurance which covers medical expenses related to COVID 19.

For those who registered with PhilHealth, ANY of the following documents shall be submitted to their colleges:

For students who are below 21 years old:

▪️ Printed copy of PhilHealth Member Data Record (MDR) of parent showing his/her name as dependent; or

▪️ Printed copy of Member Data Record (MDR) of the student enrolled as active PhilHealth member.

For students who are 21 years old and above:

▪️ Printed copy of Member Data Record (MDR) of the Student enrolled as active PhilHealth member.

Students may also secure Member Data Record (MDR) online. Kindly be guided by the following procedures:

For dependents (below 21 years old):

‣ Parents/legal guardians may visit PhilHealth website: https://memberinquiry.philhealth.gov.ph/member/ and log in to their account using PhilHealth Identification Number (PIN). If not registered, they have to create their own account by using PhilHealth Identification Number (PIN). Then, choose MDR printing button to print out the MDR Form.

For PhilHealth active members:

‣ Visit PhilHealth website https://memberinquiry.philhealth.gov.ph/member/ and log in to your account using PhilHealth Identification Number (PIN). If not registered, create your own account by using PhilHealth Identification Number (PIN). Choose MDR printing button to print out your MDR Form.

Where to submit copy of the updated MDR?

Students shall submit the printed/hard copy of MDR at their respective colleges.

FREQUENTLY ASKED QUESTIONS OF PHILHEALTH NON-MEMBERS

1. How about those students who are below 21 years old whose parents are NOT PhilHealth members?

Either parent/legal guardian should apply for PhilHealth membership to qualify them as dependent; or
Student may apply separate PhilHealth membership.

2. How about those students who are 21 years old and above and are NOT yet PhilHealth members?

He/She may apply as:

Walk-in registrant at any Philhealth Offices in Puerto Princesa City (Philhealth San Jose or Philhealth Express Robinsons place), for students located in Palawan area. Submit the ff:
• PMRF available ONLINE (https://www.philhealth.gov.ph/downloads/membership/pmrf_012020.pdf) and at OSAS office
• At least two valid IDs or a photocopy of your PSA-issued Birth Certificate.

A student may also apply as indigent member and submit the following requirements at CSWD Office:
• Certificate of Indigency from Barangay where the student resides
• Duly accomplished PMRFs
• Duly accomplished CSWD In-Take Sheet (available at CSWD office)
• Photocopy of Birth Certificate (PSA or NSO)
• Photocopy of COR
• Vaccination Card

3. How about a student who has an existing equivalent medical insurance that covers medical expenses related to COVID 19 aside from PhilHealth?

Submit the original or authenticated copy of the certificate or insurance policy with COVID 19 coverage for validation/verification.

4. What are other insurances students may avail?

Students can also avail accident insurance on an optional basis

SPOKEN POETRY CONTEST

GUIDELINES/MECHANICS
1. The contest is open to all STUDENTS OF PSU SAN RAFAEL

- NO REGISTRATION FEE-

2. Only one (1) participant in every block or Year/ Department

3. Submission of entries will be on March 16-25, 2022 until midnight. Late entries will not be accepted.

4. The concept of the spoken poetry shall be dependent on the creativity of the participant with relevance to the theme of CCRD PSU San Rafael- “ We make change work for women”

5. To those who are interested, just send your Spoken Poetry Entry to Ms. Andrea Mae Cabuniag and Mr. Jericho Belviatura

6. All the entries of Spoken Poetry shall be uploaded on official page of the Student Government CCRD San Rafael on March 26, 2022.

7. Participants shall gather reacts, comments, and share for their entries until March 29, 2022.

7.The top 3 entries will be win shall be ranked according to judges scores (70%) and their scores for the reacts, comments, and shares on the page (30%).

8.The criteria for judging of the judges will be:
Relevance to the theme- 30 %
Creativity/Style and Originality- 20%
Message and Content- 20%
TOTAL- 70%

9. The criteria for judging in reacts and shares will be:
Reactions-20%
Shares-10%

TIKTOK DANCE COMPETITION

Mechanics and Guidelines:

1. The contest is open to all STUDENTS OF PSU SAN RAFAEL

- NO REGISTRATION FEE-

2. Only one (1) participant in every block or Year/ Department

3. Music maximum time 1 to 2 minutes.

4. Using of costumes and props is allowed.

5. To those who are interested, just send your TikTok Dance Video Entry to MS: FELY RELAMPAGOS AND MR: RANRANZ MAGDAYAO BAUTISTA .

6. The TIKTOK DANCE VIDEOS will be posted on our official page (Student Government CCRD San Rafael)

7. After posting the TikTok Dance Video Entries to the page of ( Student Government CCRD San Rafael) these will be the start of counting reactions Only Heart and Wow reactions will be counted.

8. Deadline of submission of TikTok video on MARCH 25, 2022

9 Cut off of counting reactions is on MARCH 29, 2022

The contestants will be judged using the following criteria:

2 Number of Reactions(Wow & Heart Only): 20%

In conjunction with the PSU’s 50th Foundation Day and the 2nd Binturungan Festival, and as per Memorandum from the Office of the University President,
dated February 28, 2022,

Synchronous and asynchronous classes are suspended for all levels from March 1 to 3, 2022

Students are encouraged to participate in online and limited face-to-face activities of the University, and to follow all health and safety protocols.

This is a special non-working holiday nationwide per Presidential Proclamation No. 1236.

There will be no synchronous or asynchronous classes during such holidays.
