Fill out this pre-enrollment form for the 2nd semester and provide accurate and up-to-date information to ensure a smooth enrollment process.

1. Fill out the pre-enrollment form
(use your EAC email in filling out the form)

- follow additional steps indicated in the form if necessary
2. Get your clearance form (if enrolled in EACM during the previous semester) from Ms. Maria Isabel Sanchez from the dean’s office. If new/transferee, NO CLEARANCE NEEDED. If returnee, present copy of approved LOA.
3. Fill out the clearance form correctly. Make sure all account balance has been settled accordingly.
4. Once completed, look for Ms. Rhoda Macadangdang to sign the clearance form at ROOM 713/Faculty Room (Room 802).
5. Present the COMPLETED clearance form to Dean Cesar Mendoza for advising.
6. Get your pre-registration form from dean then you can pay or make downpayment for your tuition fee.
7. For those with failing grades and under Deed of Undertaking, please contact Ms. Maria Isabel Sanchez for more instructions.
8. Enrollment for School of Medical Technology is on January 12, 13, 14, and 15.
