From a key role in intensive care to recovery rehabilitation, physiotherapists are working on the frontline to help COVID-19 patients recover!

How exactly you might ask? Or what do we do?

Well, Physiotherapists roles are to help patients recover after they have been critically unwell with COVID-19. While many may test positive but minimal to mild symptoms, those being affected greatly with definitly need the help of a physiotherapist to recover faster.

Treatments involve patient assessment, goal setting as well as the prescription of mobility and rehabilitation treatments. To do this, physiotherapists work alongside a large multidisciplinary team.

Early rehabilitation and mobilisation involves helping to get patients up and moving. It is so Important to get patients up and moving and to facilitate their discharge from intensive care and hospital as quickly as possible.

Many patients who have severe respiratory failure as a result of COVID-19 need to be on a breathing machine. Some require โ€˜proningโ€™ โ€“ or turning them onto their tummy โ€“ which is a way to help people clear their lungs, as well as improve lung capacity and oxygen levels.

Also, many will have difficulty removing the sputum by coughing and that is where Physiotherapists come in - with some manual techniques and breathing exercises, it will help the patient breath better and hence get better sooner.

So, what the heck is overload actually about?

๐Ÿ‘‰ Well, overload consists of placing enough stress on cells of the body using exercise so that the body is forced to adapt by becoming stronger.

In other words, we are literally inducing micro damage to our cells so that they can repair themselves stronger.

For example, a strength athlete will train by attempting to lift very heavy weights such that his/her muscle cells sustain micro tears that will build back up in a way that will allow them to lift that weight with more
ease in the future.

Scared? Donโ€™t be.

๐Ÿ‘‰ Such damage is not dangerous if the principles of training are followed APPROPRIATELY.

In fact, itโ€™s the good kind of damage.

The principle of overload however does not only apply to muscles. Even other components of fitness such as cardiovascular endurance can be trained using overload.

๐Ÿ‘‰ A beginner marathon runner might reach fatigue after their first 5K run. However, if this is done consistently over time, the runner will adapt and be able to run a bit further and this keeps on happening until the body adapts to run the whole 42.2 km marathon.

The basis of overloading to induce adaptations rests on the process of exercising at specific intensities, volumes and frequencies.

๐Ÿ‘‰ Mostly, those intensities will lie just beyond your comfort zone but not far enough to be hazardous for health.

People who have some experience in exercising on their own will tend to be able to sense when they reach their limits. As for beginners, it is always recommended to start small and build up from there if you are alone.

๐Ÿ‘‰ However, if you have someone qualified by your side, they can very easily perform some tests that will allow them to quantify your optimum training intensity.

For example, most common tests include 1 RM for resistance training where the maximum weight one can lift for one repetition is measured and 2.4 km run for aerobic capacity. However, the latter might not be an accurate representation of VOโ‚‚ max for competitive athletes and therefore would require more advanced lab equipment such as a metabolic cart.

Hereโ€™s a quick tip on overloading for anyone performing resistance training:

๐ŸŽฏ For a program that demands 3 sets of 12-15 reps, if you can perform 16 reps onwards on the first set itself, the weight is too light to produce overload. If you reach muscle fatigue and fail at 10-11 reps on that same set, the weight is too heavy.

๐ŸŽฏ This does not necessarily mean itโ€™s dangerous. Instead, it means that the overload produced might not result in the adaptation that corresponds to your goal.

๐ŸŽฏ In other words, if you are aiming for 15 reps to improve your muscular endurance, failing at 10 reps might end up improving your muscular size (hypertrophy) instead which isnโ€™t a bad thing but is also not your intended goal at that moment.

Of course, if your program is using different concepts of periodization then itโ€™s a whole other matter. But thatโ€™s a whole other article so letโ€™s leave this topic for another time.

๐ŸŽฏ Another thing to bear in mind is that overload is a completely different concept from overtraining.

While the former is indispensable to progress into your goals, the latter will be more of a hindrance to that matter.

๐ŸŽฏ Stay tuned for my next article if you would like to know how to differentiate between the two and keep your training swift and productive.

So,

๐Ÿ“ if there is one thing Iโ€™d like you to take away from this article, itโ€™s that in order to progress and get one step closer to that fitness goal of yours, you will have to get out of your comfort zone and amp up your training intensity in a specific way such that you train for your goals instead of haphazardly.

๐Ÿ“ Such a systematic approach to your training intensity will ensure that you take the shortest route to achieve your goals instead of the scenic one ๐Ÿ›ค and in doing so, move on to your next goal in a shorter amount of time. ๐Ÿ†

After all, isnโ€™t a goal without a plan just another wish? ๐Ÿ˜‰

ARDS due to a viral infection like COVID is not an airway clearance problem. Itโ€™s a ventilation to perfusion problem due to changes in the epithelial layers of the type 1 alveolar cells and pulmonary capillary.

Normally those epithelial layers create a barrier that permits at most a trace amount of fluid into the alveoli & the space between the layers. The space between the two layers is incredibly small, ~1 cell thick. If the amount of fluid increases, ion pumps in the type 1 & type 2 alveolar cells (which produce surfactant that help reduce surface tension to keep the alveoli inflated or reinflate if they get compressed) are able to mobilize it out & into the lung microvasculature & lymphatics.

This is necessary, since if the size of that space between the capillary & alveoli increases or if excessive fluid enters into the space, it impairs the ability for gases to exchange in and out of your lungs. This means your body wonโ€™t be able to take in enough O2 or expel CO2. This means you will have problems fueling the cells in your body through oxidative respiration and keeping your blood pH normal.

Diffuse inflammation in the lungs which can occur with a viral infection, the epithelial layers separate, permits a large amount of fluid into that space and alveoli from the capillaries. If the amount of fluid becomes too high it will overtax the fluid regulatory systems & if severe enough can compress and fill the alveoli. This causes major impairments to gas exchange.

A large amount of fluid in the lungs can also make it hard for the lung to expand. If the amount of fluid gets too high, it can even leech into the pleural space causing pleural effusion making it even harder for the lungs to expand. COVID may also damage type 2 alveolar cells; decreasing surfactant & making it hard for the alveoli to stay inflated or reinflate if they get compressed & increasing surface tension.
